CCM services include care coordination, medication management, and routine phone or video check-ins. CCM aims to lower healthcare expenditures while simultaneously improving quality of life, lowering hospitalisations, and improving health outcomes for patients with chronic diseases. CCM services are frequently reimbursed by Medicare and other payers.
